The  Role
You will work as part of the Community Health team in this Monday – Friday position. Day to day duties include; providing service to clients in surrounding communities with type 2 diabetes 
Services offered at this health service; Child Health, Community health, Sexual health, Chronic Disease Management and Health education
You will be working with a small community team in Port Hedland.

The Criteria
In order to be successful in this position, the following criteria is essential: 
Division 1 AHPRA registration 
Credentialed Diabetes Educator 
National Police Clearance 
Experience working with Indigenous Clients
