The Role:
Monday- Friday (8:00AM- 4:36PM), no on-call 
– Provide clinical services to clients with chronic diseases (including assessments, screening, interventions, follow-up and evaluation)
– Provide clinical leadership to allied health and medical staff
– Assist with training and mentorship of staff when necessary
– Identify and address problems in client care plans

The Benefits:

About You:
Must have AHPRA Registration
Post-graduate Experience in Chronic Condition Management
Must have Immunization Certification
