Position Overview:

As a Clinical Mental Health Nurse specializing in Community Case Management, you will play a crucial role in providing comprehensive mental health care to individuals in the community setting. Your responsibilities will include case management, assessment, planning, and implementation of care plans tailored to each client’s unique needs.

Key Responsibilities:

  • Conducting comprehensive mental health assessments
  • Developing and implementing individualised care plans
  • Providing evidence-based interventions and therapeutic support
  • Collaborating with multidisciplinary teams and community services
  • Monitoring client progress and adjusting interventions as necessary
  • Advocating for clients’ rights and promoting recovery-oriented practices

Schedule:

This is a full-time position, Monday to Friday, with AM shifts.

Requirements:

  • Registered Nurse qualification with current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Experience in community mental health nursing and case management
  • Strong assessment, communication, and interpersonal skills
  • Ability to work autonomously and as part of a multidisciplinary team
  • Commitment to person-cantered care and recovery principles
  • Valid driver’s license and access to reliable transportation
