We are seeking an experienced and compassionate Adult Community Mental Health Nurse with a focus on case management to join our team. In this role, you will provide comprehensive care and support to adults with mental health issues, working closely with them and their families to develop and implement personalized treatment plans. You will manage client cases, monitor progress, and coordinate care to ensure optimal outcomes and improved quality of life for your clients.

Criteria: To excel in this role, you should possess:
* Qualifications: Current registration as a Registered Nurse AHPRA with no restrictions
* Experience: Proven background in mental health nursing, particularly in the community setting and case management.
* Communication Skills: Excellent communication and interpersonal abilities to engage with patients and team members effectively.
* Collaboration: Ability to work collaboratively within a multidisciplinary team.
