This opportunity is to join the Flinders and Upper North Local Health Network on a contractual basis, working fulltime Monday to Friday.

Key Responsibilities

– Support clients / carers / families across the Patient Journey, providing effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow up

– Support the effective intake, prioritization and service delivery to all client groups based on the Access Framework and the individual’s socio-economic circumstances

– Develop and apply understanding of the needs of rural, culturally diverse and Aboriginal communities

– Ensure clients/customers receive appropriate Social Work services by applying cultural sensitivity, social justice, and community participation principles

– Identify and address the clinical and individual needs of individuals within target groups within at-risk populations

– Under the Integrated Primary Health Care Program, visit and provide a care co-ordination role to clients with chronic conditions living in the region

– Contribute to the effective research, planning, coordination, reporting and evaluation of the Integrated Primary Health Care Program and other projects as required

– Utilise the Safety Learning System (SLS) to report patient risks, incidents, and client feedback

– Support the Regional Senior Social Worker and Team Leaders with administrative, clinical service improvements and other associated tasks as required

– Utilize service prioritization, eligibility criteria and tools to address clinical service demands on a need’s basis

– Collaborating with the Regional Team Leader and Senior Social Worker to deliver high quality, comprehensive and integrated Social Work services to eligible clients/groups

– Contribute to professional leadership in the application of clinical protocols and standards across the multi-disciplinary teams and the region

– Apply clinical skills to a broad scope of practice, delivering services which promote self-care and personal responsibility, and are appropriate to the local rural context

– Consistently manage a high volume and increasingly complex Social Work caseload and support other team members in managing the demands of the service

– Develop and maintain professional clinical networks within the LHN Region, and South Australia, actively sharing and seeking out knowledge of effective practice

– Active participation in the Social Work Network as required and directed

– Provide clinical support to Social Workers and professional staff within the Clinical Services Teams across the Region

– Contribute to the supervision of Social Work students on clinical placements

– Adopt a proactive approach to developing and maintaining contemporary knowledge and skills in Social Work

About You

– Tertiary qualifications in Social Work and eligible for full registration with AASW

– Previous experience in a similar role and setting; providing effective intake, assessment and triage, providing timely referrals and liaising with external providers

– Demonstrated experience and skills in community engagement, client-centred practice and cultural competency

– Well-developed communication, liaison, prioritization, problem-solving and interpersonal skills

– Participated in service planning and development, quality improvement and evaluation

– Current C class drivers’ licence and ability to travel within the region as required

Salary

– $65 per hour plus super, based on experience
