We are looking for dedicated nurses who can commit to contracts for a 12 week duration in regional and remote locations across Australia. About Mental Health Community Case Management Roles
As a Mental Health Registered Nurse specialising in community case management, your role extends beyond clinical care to include:
- Comprehensive Assessments: Conducting thorough assessments of clients’ mental health needs, strengths, and challenges.
- Individualised Care Plans: Developing, implementing, and regularly reviewing personalised care plans in collaboration with clients and their families.
- Crisis Intervention: Providing immediate support and intervention during mental health crises to ensure safety and stability.
- Medication Management: Administering and monitoring medication, ensuring adherence, and managing side effects.
- Advocacy: Advocating for clients’ rights and needs within the healthcare system and broader community.
- Home Visits: Providing care in clients’ homes, fostering a supportive and familiar environment for recovery.
- Collaboration: Working closely with a multidisciplinary team, including psychiatrists, psychologists, social workers, and other healthcare professionals.
Criteria: To excel in this role, you should possess:
- Qualifications: Current registration as a Registered Nurse with AHPRA with no restrictions.
- Experience: Proven background in mental health nursing, particularly in case management settings with a minimum 5 years of experience.
- Communication Skills: Excellent communication and interpersonal abilities to engage with patients and team members effectively.
- Collaboration: Ability to work collaboratively within a multidisciplinary team.
- Transportation: Valid driver’s license, as it’s essential for this contract role.
- Flexibility: the ability to be away from home travel nursing to regional and remote locations for a minimum of 12 weeks at a time.
