Clinical Care Manager – Home Care

This is an excellent opportunity to relocate to the beautiful Wauchope region, located approximately 4 hours north of Sydney on the Mid North Coast of NSW, and join a dedicated Home Care team making a real difference in the community. We are seeking an experienced Clinical Care Manager – Home Care to provide clinical leadership, drive quality improvement initiatives, and support the transition to the new Support at Home program.

About the Role

• Position: Clinical Care Manager – Home Care
• Employment Type: Full-time Permanent
• Salary: $120,000 base salary + Super

Key Responsibilities

• Provide clinical leadership and oversight across the Home Care program.
• Drive clinical quality improvement initiatives and strengthen clinical governance.
• Work closely with the Home Care Manager to implement and progress an approved action plan.
• Lead Support at Home readiness activities and continuous improvement initiatives.
• Mentor and support clinical staff to deliver safe, person-centred care.
• Ensure compliance with aged care legislation, quality standards, and best practice.

About You

To be successful in this role, you will have:

• Current AHPRA registration as a Registered Nurse.
• A minimum of 3 years’ Home Care management experience.
• Strong understanding of the Australian Home Care and aged care sector.
• Demonstrated experience leading clinical improvement and quality initiatives.
• Excellent leadership, communication, and stakeholder engagement skills.
• A collaborative approach with a passion for delivering exceptional client outcomes.
